Prepare for Your Visit
In preparation for your visit to Metro Urology, please make a note of the following:
- You must pre-register: FAILURE TO PRE-REGISTER COULD SIGNIFICANTLY DELAY YOUR APPOINTMENT OR RESULT IN A CANCELLATION. Please call the Business Office at (651) 999-7021 to register. Please have your insurance card(s) available at the time you call.
- Bring all insurance information: we will need to make a copy of your insurance card(s).
- Co-payments are due at the time of your visit: We accept cash, checks, MasterCard, Visa and Discover.
- Bring all recent lab work and x-ray films: Failure to do so may delay your appointment.
- Obtain a referral from your primary care physician: If your insurance plan requires a referral to see a specialist, please contact your primary care physician to verify that a referral has been issued to us for your visit. If a referral is needed and not received, it may cause a delay and possible cancellation of your appointment.
Completion of Paperwork/Forms: At the time of your visit, you may be asked to complete our registration form. For your convenience, you may complete this form prior to your visit and bring it with you. You still may need to fill out additional forms or provide further information at the time of your exam.
