Insurance and Financial Information
Please be prepared to provide your current insurance card/s at the time of your appointment to determine your payment obligation.

Insurance Card and Copay

Please remember to bring your current insurance card/s with you to your appointment and be prepared to pay any required insurance co-pays at the time of service. If you do not have insurance, we will require payment at the time of service. All patients must pre-register by calling our Business Office at (651) 999-7020­­­. Failure to pre-register could delay your appointment. Please have your insurance card/s available at the time you call. When you come in for your appointment, bring any recent lab work and x-ray films. For your convenience we accept cash, checks and major credit cards as payment.

Referrals

We know insurance details can sometimes be confusing. Many insurance companies require a referral before your visit. Prior to your appointment, please contact your insurance carrier or primary care physician to determine if a referral is needed. If you choose to be seen without the appropriate referral in hand, you agree to be responsible for the charges should they not be covered by your insurance.

Additional Information

HMO's and PPO's
HMO and PPO patients are expected to obtain a referral for the visit (if required by your plan) and to pay the co-payment specified by the plan at the time of service. Please familiarize yourself with your specific coverage to understand your payment obligation. Contact your insurance company to determine your plan's specific requirements and coverage options.

MEDICARE
Medicare patients are not required to make payment at the time of service. Any deductible or patient responsibility will be determined after billing Medicare and any supplemental insurance policies, if provided.

MEDICAID
Medicaid patients are required to bring their current Medical Assistance card reflecting coverage eligibility for the appointment date. If your plan requires a co-payment, it will be due at the time of service. If you do not have your card, we may not be able to verify your plan eligibility and you will be considered a self-pay patient.

COMMERCIAL
We will bill commercial plans. Please familiarize yourself with your specific coverage to understand your payment obligation and to verify if we are in-network for your plan.

SELF PAY
Our business office team will work with the patient and care provider to determine an estimate for the costs of the services provided for the visit. However, this is only an estimate and any remaining balance will be billed to the patient. We accept cash, personal checks, money orders, Visa, MasterCard, Discover and American Express. Self pay patients are expected to pay 100% of the bill at the time of the visit.
